Telehealth Informed Consent

Last updated 12 August 2026

Your care is provided by an independent professional group

PinwellRx is a technology and administrative platform. It does not practice medicine. Clinical care arranged through this site is delivered by independent licensed clinicians affiliated with OpenLoop Health, an independent professional group. The controlling telehealth informed consent for your care is theirs, and it governs if anything on this page differs from it.

This page is general health information, not medical advice. It cannot diagnose you or tell you what treatment is right for you. Only a licensed clinician who has reviewed your history can do that, and they may decide treatment is not appropriate. Individual results vary and no outcome is guaranteed.

What telehealth is

Telehealth is the delivery of health care using secure electronic communication rather than an in-person visit. Depending on your state and your clinical situation, your visit may be synchronous, meaning live video or phone, or asynchronous, meaning your clinician reviews the questionnaire, photographs, and history you submit and responds without a live appointment. Both are recognized forms of care.

What you are consenting to

By proceeding you consent to being evaluated and, where clinically appropriate, treated by a licensed clinician through telehealth. You consent to the electronic transmission of your health information to that clinician, to the pharmacy that dispenses any prescription, and to the platform partners that support your care. You confirm that the information you provide is accurate and complete to the best of your knowledge, including your current medications, allergies, and medical conditions.

Benefits

Access to a licensed clinician without travel, greater scheduling flexibility, privacy, and continuity of follow-up through your patient portal.

Risks and limitations

Telehealth is not equivalent to an in-person examination. Your clinician cannot physically examine you, and a condition may be missed or misjudged as a result. Technology can fail, and transmissions can be delayed or interrupted. In rare cases electronic security measures can be breached. Your clinician may determine that telehealth is not appropriate for your situation and may decline to treat you and refer you for in-person care instead. You may need laboratory work or in-person follow-up.

No guarantee of treatment or outcome

Paying for an evaluation does not entitle you to a prescription. A clinician prescribes only when it is medically appropriate, and may decline. No specific outcome is promised, and individual results vary. Where a treatment is a compounded preparation, it is not FDA-approved and the FDA does not review it for safety, effectiveness, or quality.

Emergencies

Telehealth is not for emergencies. If you are experiencing chest pain, difficulty breathing, severe allergic symptoms, thoughts of harming yourself, or any other emergency, call 911 or go to the nearest emergency department. Do not wait for a portal message.

Your rights

Your participation is voluntary. You may withhold or withdraw consent at any time without affecting your right to future care, and you may request in-person care instead. You have the right to know who is involved in your care, to access your medical record, and to have your information protected under HIPAA as described in the Privacy Policy and the Notice of Privacy Practices linked from it.
