What is the difference between semaglutide units and milligrams?

What a unit actually is

On a U-100 syringe, “units” are volume marks. One hundred units equal 1 milliliter (1 mL). Forty units equal 0.4 mL. The hash marks do not know how much medicine is dissolved in that milliliter. They only know space.

If two people both “draw 20 units,” they have matched volume, not milligrams, unless their labels list the same concentration and the same prescribed draw. A unit is not a milligram. Treating it as one is how internet conversion charts go wrong.

Where milligrams live

Milligrams live on your vial label, as a concentration (mg per mL) plus the instructions written for that fill. Compounding pharmacies can dispense different concentrations. A later refill can look different from the last one even when the drug name is the same.

Forum screenshots, group chats, and “units to mg” posts are other people’s concentrations. They are not your prescription. Do not reuse someone else’s arithmetic. Do not reuse an old fill’s unit number if the new label lists a different concentration.

Oral milligrams are a different language

A milligram in a tablet is not a milligram in a vial of injectable solution, and it is not a unit on a syringe. Different dosage forms are absorbed differently. Do not convert oral milligrams to injection milligrams or units. Do not convert an injection draw into a tablet strength. If you were prescribed one form, that label does not translate to the other.

If the numbers do not line up

Stop. Do not guess a new draw. Do not “split the difference” between an old vial and a new one. Call the pharmacy named on the label, or the clinician who prescribed, and wait for instructions that match the fill in your hand. Compounded medications are not FDA-approved. A licensed clinician can decline.

Because they are usually comparing different concentrations. Units are volume. Milligrams depend on the mg/mL printed on that person’s vial. Another patient’s screenshot is not your prescription.

Can I convert oral to injection?

No. Oral milligrams and injectable milligrams are different languages. Do not convert oral to injection or injection to oral from this page or from a forum post.

The new vial looks different. What should I do?

Stop and call the pharmacy or the clinician. A new concentration means the old unit number may no longer match the old milligram amount. Do not reuse the previous draw because it “looked close.”
